TMJ is one of the more frustrating things to carry, partly because of where it sits. Dentists tend to see the teeth and the guard. Doctors tend to see the headache. The joint itself, the muscles working it, and the nerves running through the area belong to all three conversations and get owned by none of them.

Why a night guard alone does not settle it

A guard protects your enamel. It does not stop you clenching. If the muscles pulling on that joint stay locked, the jaw pain, the temple headaches, and the neck tension keep going, and you keep waking up tired from a night of grinding.
