The exam looks at how the whole leg loads. A hip that has lost rotation, an ankle that stopped dorsiflexing after an old sprain, a foot that collapses on one side more than the other. Any of those puts a knee under stress it was not built for, and no amount of treatment aimed at the joint itself changes the reason it keeps getting angry.
Wear on a scan is not the whole story
Plenty of people arrive with imaging showing arthritic change and advice to manage it until a replacement makes sense. The wear is real. It also does not account for all of what you feel. Muscles compensating around a stiff joint and a nervous system that has been guarding the area for years make up a share of the pain, and those parts respond even when the joint surface does not change.
